Terms & Conditions — Evolve IQ Healthcare
Effective Date: November 20, 2025
1. Acceptance
By accessing our website or using our services, you agree to these Terms and any applicable
Statement of Work (SOW) or Business Associate Agreement (BAA).
2. Services
We provide the following professional services:
- Revenue Cycle Management (RCM)
- Medical Billing
- Coding & Auditing
- Accounts Receivable (AR) follow-up
- Denial management
- Prior authorization
- Eligibility verification
- Reporting & Analytics
3. Client Responsibilities
Clients agree to:
- Provide accurate and complete patient or practice data
- Ensure necessary patient authorizations
- Maintain their own HIPAA compliance requirements
- Respond promptly to requests for information
4. Privacy & Security
We follow HIPAA regulations and execute Business Associate Agreements (BAAs) when applicable.
All data shared with us is handled in compliance with federal and contractual requirements.
5. Fees
All fees, payment schedules, and billing terms are defined in the executed SOW for each engagement.
6. Confidentiality
Both Evolve IQ Healthcare and the Client agree to protect all confidential business, financial,
and operational information received during the engagement.
7. Liability
Evolve IQ Healthcare’s liability is limited to the total amount of fees paid by the Client in the
previous 12 months.
8. Termination
Upon termination, any PHI or confidential data will be returned or securely deleted in accordance
with HIPAA and contractual requirements.
9. Governing Law
The governing law shall be the jurisdiction specified in each applicable SOW.
10. Amendments
We may update these Terms periodically. Continued use of our services constitutes acceptance of any revised Terms.
